The U.S. Senate rejected competing bills to extend Affordable Care Act subsidies, paving the way for steep premium increases for millions of Americans starting January 1.​

Democrats pushed a three-year extension of COVID-19-era enhanced tax credits, but the measure failed 51-48, short of the 60 votes needed. Four Republicans—Sens. Susan Collins, Josh Hawley, Lisa Murkowski, and Dan Sullivan—joined Democrats in support. A Republican alternative offering health savings accounts up to $1,500 also fell 51-48.​

The lapse affects about 22 million on ACA marketplaces, with premiums potentially doubling or tripling in many cases. Senate Democratic Leader Chuck Schumer warned of no further chances before costs rise. Republicans argued subsidies fail to address skyrocketing premiums from systemic issues.​​

With Congress nearing holiday recess, the impasse heightens health care affordability debates ahead of midterms.
